Abstract

Arthroscopic hip surgery for femoroacetabular impingement (FAI) correction and labral preservation, presents intricate challenges necessitating precise tools, surgeon expertise and experience for optimal outcomes. The emergence of articulating rotary burs (ARB) offers the potential to revolutionize surgical accuracy, precision and learning curves in arthroscopic hip FAI correction due to its remarkable 601% increase in cutting surface area compared to traditional burs. In this video presentation, we explore the transformative possibilities of this emerging technology in arthroscopic hip surgery.

Through a comprehensive review of current literature, clinical case studies, and arthroscopic video examples, we elucidate the key advantages articulating rotary burs possess to optimize more accurate FAI correction while making the learning curve less daunting for newer surgeons. We also delve into the mechanism behind ARB’s increased cutting surface area, highlighting its implications for more accurate bone resection in hard to reach areas and capsular preservation. Due to their unprecedented small radius of movement, articulating rotary burs aim to minimize capsular and soft tissue disruption during FAI correction, empowering surgeons to navigate complex anatomical structures with unmatched accuracy.

The educational merit of this presentation lies in its focus in helping surgeons gain a nuanced understanding of how to leverage this future technology to optimize surgical approaches, minimize complications, and allow more patients to benefit from arthroscopic hip preservation surgery. With any new technology, we underscore the importance of adopting an evidence-based approach, fostering a culture of continuous improvement and innovation in surgical care.

Learning objectives:

  1. Understand the fundamental principles underlying the design and function of articulating rotary burs in arthroscopic hip surgery

  2. Evaluate the clinical significance of articulating rotary burs in femoroacetabular impingement (FAI) correction and labral preservation

  3. Identify strategies for leveraging the increased cutting surface area of articulating rotary burs for improved efficacy and efficiency during cam resection, subspine decompression, and acetabular preparation.

In conclusion, this video seeks to demonstrate articulating bone resection technology for arthroscopic hip surgery and highlight its potential for enhanced precision, improved patient outcomes and continued innovation in joint preservation surgical techniques.
